hi everyone, ive been to the dentist quite a bit recently due to pain with my wisdom teeth.
i need to have them extracted and was wondering if i had to have any special medication to have this done as i will have to have it done on the nhs.
i have type 1 vwd and so far the only problems i have had are with severe bruising, nose bleeds and bleeding gums!! im quite nervous to have it done any advice/ support would be welcomed!

hi. i had all my wisdom teeth extracted in hospital about 10 years ago.i was given a general anaesthetic and also had clotting factor but not sure what it was.i had 2 stay in overnight.after the extraction i had minimal bleeding/bruising and used a medicated mouthwash at home. I was nervous too but it was fine!good luck!
It’s important to tell the dentist you have vWD. Extractions can cause major bleeding even in a non vWD person. My guess is they would probably want you on something like amacar for a few days until the clot is stable. Our hematologist always says that if my daughter who is type 1 is going to have dental extractions, she needs her amacar and Stimate. You should definitely have some sort of clotting factor for the surgery. I wouldn’t get too nervous though…with a clotting factor you will be fine.
Hi I had one of mine out quite a few years ago and they gave me the Desmopression IV before. It was done in the specialists office as it was set up with nurses and everything they would need. I definitley would not get them out without getting in touch with your blood specialist. Now I keep forgetting what type of VW I have. I did have my cheek bruise and swell up like an overfed chipmunk. Anytime I have had any kind of major dental work I have had the Desmopression before. I think you have an hour window for the work to be done after getting it and may have to have more later in the day depending on what type you have. Hopefully this little bit helps.
Interesting conversation I stumbled upon…I am a dental assistant… I have VWD… And I just had my wisdom teeth extracted less than two years ago. So… You need to tell the oral surgeon and you need to get clearance from your oncologist… If you don’t have an oncologist… Get one! I seemed to have suffered the same side effects as you… I had to do infusions to get all my levels in the upper range to prepare my body for the stress. Otherwise all went well. Talk to your oncologist, talk to your oral surgeon, let your oral surgeon and oncologist talk… I did all of these, I however did find an oral surgeon who was also an MD as well as a DDS. The medication has improved since I had my wisdom teeth taken, although I've had a C-section 2 times with type 1 VW and I just took Humate P and stimate. I think they would just have you take the stimate nasal spray before and after on certain days. There are certain days after surgery that the wound is more prone to break and bleed. I think it is days 3 & 4.... not certain. See a Hemotologist. You will be fine, just need to right meds to take precaution.

Im having general with factor 8 before and possibly after. I have type 2B. My son had his out about 4 years ago -(2B also)and he was fine. There seems to be some problems for me with the breathing tube down my nose as they are worried about bleeding. Im nervous but want the teeth out as the pain is becoming unbearable.

I have acquired VWD. I have immune system antibodies which attack my VW factor and factor eight. My Hematologist had me in the hospital for GI bleeding and tested the efficacy of DDAVP (Stimate/desmopressin) to raise my VW factor. The test was a VWF test before the administration of DDAVP and then a second test timed, exactly, two hours later to see what effect the DDAVP had on my VW factor in the blood. I went from less than 5% to close to 20% of what is normal. His opinion was that DDAVP would handle a small nosebleed or gum bleed but VWF/factor eight concentrate (HumateP or Alphanate) would be required for any significant surgery. Even with Alphanate, I had significant bleeding with surgical implantation of a cathport. (which resulted in a MRSA infection and had to be removed with more Alphanate pre-treatment. Unless your doc knows how well DDAVP or Amicar works in your specific case, test first is my advice.
The Hospital staff had a great deal of difficulty understanding the test protocol and if I hadn’t known of the importance of the correct timing of the blood draws relative to the DDAVP IV, the test would have been fouled up and useless. Keep a close eye on hospital staff with this condition.
I also had a run-in with an IV tech when they were giving me IV Vancomyacin for the MRSA infection.
Heparin IV rinses are standard procedures for IV ports, cathports and PIC lines. Do not let a hospital worker come near you or your IV with a yellow tipped syringe. It contains a strong anti-coagulant which will increase your chance for bleeding. Fight them off. Yell lawyer. In your condition a normal saline rinse is more than adequate and is standard procedure for VWD and Hemophilliac patients.

The Stimate spray is a factor sprayed in the nose. It eloped raise my daughters levels tremendously. However they said Stimate doesn’t help to stabilize the clot. We also have a nasal amacar spray and she has taken liquid amacar. They said the amacar stabilizes the clot vs just stopping the bleed. Had Stimate nt worked my daughter would have to do the humate infusions which s an actual blood product.
I went to an oral surgeon. They ran an IV of some drug to stop bleeding before I had them pulled. And they did put me under.. but the doc said after the surgery, I had little to no bleeding because of the IV. He joked and said I wish I could give that to all of my patients.. My recovery was fine.. no bleeding.. I have Type I also.
